APPLICATION FORM
Data subjects defined as "concerned persons" in the Personal Data Protection Law No. 6698 ("PDP Law") (hereinafter referred to as "Applicant") have been granted the right to make certain requests regarding the processing of their personal data under Article 11 of the PDP Law.
Pursuant to the first paragraph of Article 13 of the PDP Law, applications to be made to our Company, as the data controller, regarding these rights must be submitted to us in writing or through other methods determined by the Personal Data Protection Board ("Board").
In this framework, applications to be made to our Company "in writing" can be submitted to us by printing out this form through:
- In-person application by the Applicant,
- Filling out the application form and sending it using mobile signature or by using the fax number previously notified to the data controller by the concerned person and registered in the data controller's system.
Below, information is provided specific to written application channels regarding how written applications will be delivered to us.

Your applications submitted to us will be responded to within thirty days from the date your request reaches us, depending on the nature of the request, in accordance with paragraph 2 of Article 13 of the PDP Law. Our responses will be delivered to you in writing or electronically pursuant to Article 13 of the PDP Law.
The use of this form for applications is not mandatory. This form has been created as a supporting document. Every application made in accordance with the communiqué on the procedures and principles for applications to the data controller will be evaluated.
