Application Form
Under the Turkish Personal Data Protection Law No. 6698 (“KVKK”), personal data subjects (hereinafter referred to as the “Applicant”) are granted certain rights to make requests regarding the processing of their personal data under Article 11.
Pursuant to the first paragraph of Article 13 of KVKK, applications to our Company, acting as the data controller, must be submitted in writing or through other methods determined by the Personal Data Protection Board (the “Board”).
Accordingly, written applications to our Company may be made by printing this form and submitting it via the channels below;
- ✓ By applying in person (the Applicant applies personally),
- ✓ By completing the application form and sending it with a mobile signature or via a fax number previously notified by the data subject and recorded in the data controller’s system,
These applications can be delivered to us. Below, information is provided on how written applications can be submitted through specific channels.

Your application will be responded to within thirty (30) days from the date it reaches us, depending on the nature of the request, pursuant to paragraph 2 of Article 13 of KVKK. Our responses will be delivered to you in writing or electronically in accordance with Article 13.
Using this form is not mandatory. This form has been prepared as a supporting document. Any application submitted in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller will be evaluated.
